Compassionate Mental Health Care

 To establish care at Northern Lights Psychiatry, the following steps are required:

1. Inquiry/ Pre-screen Form – Complete and return a brief pre-screening form so we can determine the best fit for your needs.
    
2. Insurance Verification – Please confirm with your insurance provider whether we are an in-network provider.
    
3. Intake Packet – Once your request is received, a team member will contact you. You will then complete a detailed online intake packet, which provides your medical and psychiatric history.
    

• All new client paperwork must be completed at least (2) days before your appointment.
   
• Insurance and payment method to be on file (2) days before scheduled appointment

• Initial evaluations are typically 60 minutes in length.

• If your visit is via telehealth, you will receive a secure link prior to the appointment
   
• No medication will be prescribed until the full evaluation is completed.
   
• We understand that first appointments can feel stressful; our goal is to welcome you into a calm, supportive, and relaxed environment.
   
• Please note: We do not provide services for legal cases, court testimony, or domestic disputes. Our role is to diagnose and treat mental health conditions.
